Level1,Level2 outlined heading are not appearing in .chm

Hi,
I have a preexisting document which i need to convert it into .chm.
I am using a trial version of Word-2-CHM converter.
As per instructions, i defined Level1, Level2 etc and also the body text in ouline view.
Then used the converter to generate .chm file.
But the following problem occurs after conversion
1. Only body text is appearing on right side window.
2. Sub and SUb-Sub heading are completely disappeared.
3. And every heading is considering as a Level1, which result in separate page for every topic.
Please let me know, what wrong am i doing?
For our project , we are planning to buy a license version of doc-2-chm converter.
But before buying a registered version , we need to clarify this query.
Can you please resolve the queries.
As it's little bit urgent, we would appreciate a quick response from your side.
