If I manually enter an index term, and place it in quotes, Word tends to use "smart quotes" instead of the plain old ascii quotes. These show up fine in the Word document index, but do not show at all in the generated help index, or sometimes generate blank entries pointing to the first topic.
version: Pro, 2009.2.708.1861.
