Strange structure of topic tree in generated CHM file

We are using Word-2-CHM 2007 Pro to generate a Help file from our product documentation written in Word.
As a preliminary step to the conversion with Word-2-CHM, we save the Word document in .mht format.
Now, when we generated the Help file back in February 2009, using Word-2-CHM 2007.13.912.662 on my machine running Windows XP and Word 2007, the structure of the generated topic tree was ok, reflecting the Word document's table of contents.
When we generate the Help file now, still using Word-2-CHM 2007.13.912.662 on my machine, but running Windows Vista and Word 2007, the structure of the generated topic tree looks really strange, e.g. there are top-level entries with text from the cover page, or references of figures and tables.
Note that we didn't change the .word2chm and .mht file!
What can we do the regain the topic tree structure reflecting the Word document's table of contents?

Does the original .doc exist?
If not, please try to open the .mht in Word and save it as .doc and try again.

Yes, the original .doc exists, but using the original .doc instead of the .mht generates the same weird results.
Also saving the .mht file as .doc in Word and using it to generate the .chm file doesn't help.
The structure in both .doc files and the .mht file is OK.

Sorry for the delayed response.
Did you copy the generated chm help from the Vista to the XP and review the result?

Same weird results on XP. We even tried the whole conversion process on XP. No change.

The .mht format is
not supported
by Word-2-CHM. Please save the .doc as “Web File”.
Do not save it as “Single File Web Page” or “Web Page, Filtered”.

Hm, then, I am wondering why I can select .mht files as source file...
Nevertheless, it doesn't matter if I select the original .doc file, the .mht or the .htm file, the weird results remain the same.

Problem solved.
We fixed the outline level of the paragraphs that occurred in the Document Map by mistake and Word2chm now generates a .chm file with an appropriate topic tree.
